This report can be completed on the Pennsylvania State Police website, https://epatch.state.pa.us. You will pay a fee of $22.00 for the online record check, which will display a summary listing of the Record Check Results. Click on “Certification Form” to access a printable certificate validating that a record check was conducted.  Please note that the Purpose For Request must be “Employment”.
