Transition Counselors in the Pittsburgh Public Schools provide counseling and guidance services to students ages 14-21 with disabilities. In addition to the typical responsibilities of a guidance counselor, transition counselors are responsible for the transition requirements under IDEA and serve as an integral part of the student’s transition from high school to adult life. Transition Counselors coordinate transition activities and services such as administering transition assessments, developing, writing and implementing transition plans, coordinating services both within the school as well as providing a linkage to appropriate adult agencies. These services help to prepare students with disabilities for employment, post-secondary training and independent living.
