These are the current transportation forms.

Parent Transportation Log
The Board of Directors approved mileage reimbursement to families eligible to drive their child to and from school for the 21-22 school year. Eligible families will receive communications if the District cannot provide transportation, asking them to release their seats. An eligible student is a student that generally would receive yellow bus services, and the District is unable to provide the service at this time.

Joint Custody Guidelines
This information explains the Districts Transprotation guidelines for joint custody.

Joint Custody Transportation Calendar
A calendar for the first month of service must be submitted through the school, along with the required joint custody documentation, to Pupil Transportation. Subsequent calendars may be submitted by the school to the carrier.

Medical Transportation
This form must be completed by the student's doctor and submitted to Health Services.

Hazardous Route
This form is to be submitted to the student's school. Final determination is made by PennDOT. -
Stop Change Request
This form is to be submitted to the student's school. All decisions will be based on current Pittsburgh Public Schools and State of Pennsylvania guidelines.
